Position To assist the Director of Nursing in the planning, organizing, developing and directing of day to day functions of the Nursing Department in accordance with current federal, state, local standard guidelines and regulations that govern our Facility. Position Accountabilities and Performance Percent of Time The following are essential job accountabilities: 2. Makes daily rounds of the nursing units to ensure that all nursing staff are 15% performing their work assignments in accordance with acceptable nursing standards. Application of Knowledge Performance Criteria Provides the Director with information relative to the nursing needs of the residents and the Nursing Departments ability to meet those needs. Management Performance Criteria Authorizes use of restraints when necessary and in accordance with established policies and procedures. Ensures that nursing staff follow established departmental policies and procedures, including appropriate dress code. Ensures that work areas are maintained in a clean and sanitary manner. HIPAA Competency- Protects resident's privacy by insuring any items with resident data on them are not left unattended. Record Keeping Performance Criteria Makes written and oral reports/recommendations to the Director of Nursing as necessary/required, concerning the operations of the Nursing Department. Associate Director of Clinical Services (continued) 3. Performs administrative duties such as completing medical forms, reports, 10% evaluations, studies, charting, etc., as necessary. Application of Knowledge Performance Criteria Participates in surveys made by authorized government agencies. Participates in the selection of residents for admission to the facility as needed. Collaboration Performance Criteria Assists in developing methods for coordinating nursing services with other resident services to ensure the continuity of residents total regimen of care. Resident Relations Performance Criteria Assists with the development of safety standards for the Facility. Reviews complaints and grievances made by residents and make written report to the Director indicating what actions were taken to resolve the complaint or grievance, if necessary. 4. Participates in the development, maintaining and updating of written policies 6% and procedures that govern day to day functions of the Nursing Department. Management Performance Criteria Ensures that all Nursing Service personnel are following their respective job descriptions. Participates in developing, maintaining and periodically updating written job descriptions for each level of nursing. Ensures that nursing staff are knowledgeable of residents rights and that they are treated with kindness, dignity and respect. Collaboration Performance Criteria Serves as a resource for unit managers and supervisors regarding clinical, personnel and policy/procedures issues. Assists the Director in determining the staffing needs of the Nursing Department. 5. Responsible for assessing and monitoring skin issues throughout the facility. 10% Application of Knowledge Performance Criteria Ensures the development, implementation and revision of written standard and transmission based precautions. Ensures the development, implementation and of the infection control program for monitoring communicable and/or infectious diseases among the residents Insures that proper treatment, use of equipment and follow through is Record Keeping Performance Criteria Submits weekly pressure and ulcer report. Submits a monthly infection control report and notifies the NYS Department of Health if infections are above baseline or there is an infectious outbreak. Associate Director of Clinical Services (continued) 6. Serves on and participates in and attends various committees of the facility 6% including but not limited to infection control and quality assurance. Application of Knowledge Performance Criteria Attends and participates in developing the agenda for care plan meetings as necessary. Evaluates and implements recommendations from established committees as they pertain to the Nursing Department. Collaboration Performance Criteria Assists the Director in preparing written and/or oral reports of the nursing service programs and activities to submit to these committees. 7. Performs a variety of duties related to safety. 4% Application of Knowledge Performance Criteria Facilitates investigation of medication errors and maintains a medication error log for each nurse. Management Performance Criteria Provides investigation of and follow up on incident/accident reports. Monitors resident care and care plans to be sure that care plans are adjusted when falls occur. Collaboration Performance Criteria Assists the Environmental Director in developing a preventative program for maintaining the nursing departments equipment and supplies in a safe and operable manner. 8. Attends all inservices and meetings as required. 4% Application of Knowledge Performance Criteria Competency: Participates in drills and demonstrates knowledge of Plan for Department. Projects poise, authority, and a positive image. Wears appropriate clothing and accessories, giving proper attention to personal hygiene. Wears Facility ID Badge while on duty. Infection Control Competency: Demonstrates knowledge of and adheres to all infection control procedures including proper hand washing techniques, contact with blood spills and universal precautions. Attends department meetings as required and scheduled. Attends all mandatory inservices. Attends Unit Manager meetings, BMARC meetings, and other meetings as needed. Participates in Quality Assurance Program as required by Director The following duties may be reassigned: 9. Participates in the implementation of the facilities quality assurance program 6% for the Nursing Department. Application of Knowledge Performance Criteria Demonstrates good judgement and uses common sense in making decisions. Cooperatively acts as liaison with other departments of the facility, physicians, families, residents needs and issues. Associate Director of Clinical Services (continued) Page 4 of 8 Effectively identifies predictable problems and adjusts available resources to facilitate positive learning. Ensures compliance with quality improvement policies and procedures. Contributes daily to updating the nursing care plan. Documents significant assessments to update resident records and charts. 10. Provides direct nursing care as necessary. 5% Application of Knowledge Performance Criteria Exhibits expertise and skills in meeting needs of the residents. Follows established protocols for medication administration. Communications Performance Criteria Maintains excellent relationships with staff. Management Performance Criteria Available for any additional tasks when called by the Director of Nursing. 11. Rotates Nursing Administrative coverage as deemed necessary by the 4% Director of Nursing. Application of Knowledge Performance Criteria Assist with daily operation of units when able. Available for any additional tasks when called by the Director of Nursing or the Assistant Director of Nursing. Communications Performance Criteria Monitors communication systems which promote cohesiveness, team participation and excellence of services. Maintains excellent relationships with staff of the Hospital and the Nursing Home. Management Performance Criteria Available for any additional tasks when called by Director of Nursing or Assistant Director of Nursing. Collaboration Performance Criteria Cooperatively acts as liaison with other departments of the facility, physicians, families, residents needs and issues. Registered Nurse
