Job Summary: Responsibilities will include working with participants who are in need of academic functioning level upgrades, High School Equivalency diploma, assistance with career and employment, and/or entrance into college, vocational training programs, or military. Will conduct initial in-take and comprehensive assessments. Determine eligibility for enrollment into funded programs. Will act as a conduit for the flow of information related to referrals from community organizations, agencies, courts, probation, and school districts for the purpose of establishing and maintaining an effective network specific to case management services. Act as a positive team member and develop working relationships with peers and community partners. Essential FunctionsDevelop working knowledge of school districts and justice systems throughout Erie CountyLearn and utilize program specific data tracking system (OSOS)Determine eligibility for enrollment into funded programsAssist with the completion of paperwork necessary for entrance into training programs or college that includes admissions applications, FASFA forms, PELL grants and scholarshipsPresent the Department's programs to schools, CBOs, and other community organizationsRespond to consultation requests from educators, employment counselors, participants, and familiesPerform follow-up services as specified by program contractsProvide crisis intervention, assessment, and referral to participants on scheduled or walk-in basisConduct interviews, counseling and/or mentoring to assist individuals with gaining insight into causes of personal needs and promotion of personal developmentEngage in professional development and in-service trainingsServe on agency committees as requestedEducation and Experience: Minimum of a BA in social work or school counseling, guidance, education, or related human services.
