Responsible for providing social work services which include crisis intervention, individual, family and group counseling to patients and families.Works in collaboration with physicians and other health care personnel in patient evaluation and treatment, to further their understanding of significant social and emotional factors underlying patientA?s health problems.Links patient/family to appropriate community resources including information/referral to sources of financial assistance, transportation, food, housing, support groups.Provides short:term counseling to identify problems and facilitate needed referrals. Provides referrals for long:term counseling and for other necessary services (e.g. substance abuse treatment, legal services, housing assistance).Serves as patient advocate and liaison with physicians, families and community to ensure continuity of care.Documents patient/family status and history, diagnosis, treatment plan, evaluation results, observations and progress in medical record.Serves as a resource to other staff on community resources and in area of specialty.Assures social services compliance with legal, regulatory and professional standards, including Article 28 and JCAHO (where applicable) and OMH.Coordinates child abuse reporting for the Center.Participates in development and implementation of the departmentA?s performance improvement activities.Performs other job:related duties as required.Required:New York State LCSW LicenseFive years experience in social work with broad background with diverse patient populations (i.e., HIV, homeless, mentally ill, elderly, children adolescents, etc).Technical knowledge of medical social work and relevant community resources, and the ability to develop linkage with community organizationsAbility to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions.Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditionSkill in proper use of tools such as the DSM V:R for identifying clinical symptomsShould be comfortable with the pace of primary care, working with an interdisciplinary team and have strong communication and human relation skillsKnowledge of the development and maintenance of information/referral systemsHealthcare and mental health experience
